Our New Client & Patient Forms
12/9/21: We are not currently booking appointments for new clients. We hope to resume welcoming new clients to our practice as soon as we can, but due to a high volume of pets needing care, we must pause new clients to ensure that our established patients are able to receive timely care. Thank you for understanding.
Prior to your visit to Kalamazoo Animal Hospital, please fill out a New Client Form. If you are unable to fill out your new client form before your appointment, please arrive 5-10 minutes early to allow time for the form to be completed before your scheduled appointment time.
If your pet has had medical care elsewhere, please e-mail past records to [email protected] or fax them to 269-381-6431. If you do not have copies of your pet's records, please fill out a release form so we can request the records from your pet's previous veterinary office. If your records cannot be sent ahead of the appointment and you are bringing paper copies to the visit, please arrive 10-15 minutes early to allow us time to review your pet's records. Thank you!
During times of high demand for appointments, we may ask new clients to provide a deposit in the amount of the exam fee to hold their appointment slot. If we have asked that you pay a deposit, please use this form to read our deposit policy and provide payment information. If demand is especially high, we may be temporarily unable to accept new clients. We recommend that you contact us by calling 269-381-1570 or emailing [email protected] to confirm we are accepting new clients prior to registering or requesting an appointment.
